Summary
There is controversy about the treatment of metastases of neuroendocrine tumors (MNT). From 1994–1997 we treated 10 patients with MNT. The primary tumors were located in the pancreas (n = 3), ileum (n = 5), stomach (n= 1) and bronchus (n= 1). Seven patients showed MNT even at first diagnosis. Resection (R), chemoembolization (CE), octreotide (O) and interferon, chemotherapy or liver transplantation (LTx) were treatment modalities. One patient was cured with R, CE, O and LTx; in one patient a tumor rest was detected in the femur; four patients are alive with advanced tumors (median survival time 28 months); and four patients died. Due to a lack of randomized comparisons evaluation is limited. The therapy modalities should depend on the patient’s age, symptoms, histology, and tumor size. The resection of the tumor for curative and palliative intent is safe, provides effective palliation, and probably prolongs survival.
Zusammenfassung
Die Behandlung von Metastasen neuroendokriner Tumoren (MNT) wird kontrovers diskutiert. Von 1994–1997 behandelten wir 10 Patienten mit MNT. Die Primärtumoren waren im Pankreas (n = 3), Ileum (n = 5), Magen (n= 1) und Bronchialsystem (n= 1) lokalisiert. 7 Patienten hatten bereits bei der Erstdiagnose MNT. Als Therapieverfahren wurden eingesetzt: Resektion (R), Chemoembolisation (CE), Octreotid (O) und Interferon, Chemotherapie und Lebertransplantation (LTx). 1 Patient wurde kurativ behandelt (in zeitlicher Reihenfolge durch: R, CE, O, LTx), bei einem Patienten findet sich ein Tumorrest im Femur, 4 Patienten leben mit fortgeschrittenen Tumoren (mittlere Überlebenszeit 28 Monate), 4 Patienten starben. Die Therapie richtet sich nach Alter, Klinik, Histologie und Ausdehnung, wobei die Tumorresektion in kurativer und palliativer Intention das Verfahren der ersten Wahl darstellt. Aufgrund fehlender randomisierter Vergleiche ist die Bewertung eingeschränkt.
