Summary

Nowadays, minimal invasive operating techniques are an essential part of adrenal gland surgery. Meanwhile four different methods are being used: laparoscopically in the supine or lateral position or retroperitoneoscopically as a prone or lateral method. Until now, it is not evident whether laparoscopy or retroperitoneoscopy should be preferred. Laparos-copy offers a familiar anatomical exposure but requires 4–6 trocars. For the retroperitoneoscopic methods only three trocars are needed, whereas orientation in the operating field is more difficult. All endoscopic procedures to the adrenal gland demand high surgical skills. They require expended experience in minimal invasive surgery as well as special knowledge in endocrine surgery.

Zusammenfassung

Die minimal-invasiven Operationstechniken sind heute fester Bestandteil der Nebennierenchirurgie. Inzwischen kommen vier verschiedene Methoden zur Anwendung: laparoskopisch von vorne und von der Seite sowie retroperitoneoskopisch von hinten und von der Seite. Nicht entschieden ist, ob bevorzugt laparoskopisch oder retroperitoneoskopisch vorgegangen werden sollte. Erstere Zugänge bieten einen gewohnten operativen Situs, erfordern aber 4–6 Trokarzugänge. Die retroperitonealen Methoden kommen in der Regel mit 3 Trokaren aus, dafür ist die Orientierung im Operationsgebiet schwieriger. Unabhängig vom Verfahren gehören endoskopische Eingriffe an der Nebenniere zu den technisch anspruchvollen Operationen, die neben den speziellen Kenntnissen der endokrinen Chirurgie auch große Übung in der minimal-invasiven Methodik erfordern.
