Zusammenfassung

Obwohl die chronische Alkoholabhängigkeit eine schwerwiegende psychische Erkrankung mit Neigung zu Progredienz und zum Rezidiv ist, gibt es — neben der klassischen, hochschwelligen, mehrmonatigen stationären Entwöhnungsbehandlung — wenig allgemeine akzeptierte Verfahren zur Rückfallprophylaxe. Vor dem Hintergrund neuer Forschungsergebnisse über die biologischen, psychologischen und psychosozialen Entstehungsbedingungen der Abhängigkeit hat sich in der Therapie des Alkoholismus ein Paradigmawechsel vollzogen. So fuhren Rückfälle während der Therapie nicht mehr automatisch zum Abbruch oder zum Rückversetzen an den Behandlungsbeginn, sondern zu einer intensiven therapeutischen Bearbeitung der intrapsychischen Situation vor und nach dem Rückfall unter Berücksichtigung moderner Craving-Konzepte. Zur Zeit stehen die unterschiedlichen therapeutischen Strategien (stationäre Entwöhnung, psychosozial orientierte ambulante Behandlung, medikamentöse „Anti-Craving“-Therapien und verhaltenstherapeutische Interventionen) kaum hierarchisiert nebeneinander. Da der größte Teil der Patienten ausschließlich vom Hausarzt behandelt wird, muß im Versorgungsbereich eine Vernetzung der Kliniken mit den komplementären Diensten und den niedergelassenen Ärzten in regionale Verbundsysteme realisiert werden.
