Zusammenfassung

In den letzten zwei Jahrzehnten hat das Interesse für das Auftreten zentral nervöser Manifestationen bei malignen Lymphomen und Leukämien deutlich zugenommen. Dies ist auf die deutliche Zunahme der Incidenz-Beteiligung des Zentralnervensystems (ZNS) sowohl bei malignen Lymphomen (11, 14, 15, 48) als auch bei Leukämien (10) zurückzuführen, wenngleich bei akuten Leukämieformen durch die ZNS-Prophylaxe in den letzten Jahren wieder eine deutliche Abnahme der Incidenz der ZNS-Manifestation erzielt werden konnte (12). Als Grund für die Incidenz-zunahme wird vor allem die Einführung einer effektiveren Systemtherapie angesehen, woraus eine deutliche Zunahme der Überlebenszeit und somit der Beobachtungszeit resultiert (10, 15). Eine weitere Ursache dürfte in der verbesserten Diagnostik, vor allem hinsichtlich der Identifizierung maligner Zellen im Liquor liegen (8). Das vermehrte Auftreten einer ZNS-Beteiligung zwang auch die Kliniker und nicht, wie bisher, vorwiegend die Neuropathologen zu einer intensiveren Auseinandersetzung mit der Problematik.
