Zusammenfassung

Anastomosen zwischen dem Endteil des D. thoracicus und der V. jugularis interna zur Therapie der portalen Hypertension sind umstritten. Dieser Eingriff findet seine Begründung darin, daß auch die Leberlymphe über den D. thoracicus drainiert wird und daß dieser ein Spiegelbild der portalen Druck- und Volumenverhältnisse darstelle (Schreiber et al., 1968). Die Erkenntnis, daß bei der Leber-cirrhose ein gesteigerter Lymphfluß besteht, geht auf Starling (1894) zurück. Dumont und Mulholland wiesen 1960 einen auf das Drei- bis Fünffache gesteigerten Flow im D. thoracicus bei Lebercirrhose nach und konnten dies bei wiederholten Untersuchungen reproduzieren (Dumont U. Witte, 1966). Ein bestehender Ascites besserte sich oder ging ganz zurück.
