Abstract

Different factors may influence the decision of whether to perform a single-bundle (SB) or double-bundle (DB) ACL reconstruction. The size of the knee may play the most important role [11, 16, 18]. The smaller the knee, the more difficult it is to perform an anatomically and technically correct DB ACL reconstruction. The required ap length for DB measured at the tibial and femoral insertion sites was suggested to be 14mm or more – also depending on the diameters of the two grafts [11, 16, 18]. In case of large grafts, more space is needed to perform a DB ACL reconstruction. The “insertion site table” (see Chap. 22) displays the size of the insertion site needed to perform a SB or DB ACL reconstruction. It can be measured preoperatively on MRI or – more reliable – intraoperatively using a ruler [11, 16, 18].
