Zusammenfassung
Derzeit erfolgt eine einheitliche Diagnosestellung und Klassifizierung der akuten Nierenschädigung nach KDIGO-Kriterien: schon geringe Kreatinin-Anstiege von >26,5 µmol/l (bzw. >0,3 mg/dl) identifizieren Patienten mit akuter Nierenschädigung. Somit ist die Diagnose „Akute Nierenschädigung“ bisher an den Anstieg von Filtrationsmarkern wie Kreatinin gekoppelt, die lediglich die Funktion der Niere bewerten. Aktuelle Empfehlungen gehen dahin, auch Marker der Gewebsschädigung (z. B. NGAL, KIM-1 oder TIMP-2/IGFBP7), die oftmals der Funktionseinschränkung der Niere vorausgehen, in die Diagnosekriterien der akuten Nierenschädigung mit aufzunehmen.
Um die Sichtbarkeit von relevanten Kreatinin-Anstiegen und damit die Sichtbarkeit und Wahrnehmung der akuten Nierenschädigung bei Krankenhauspatienten zu erhöhen, ist die EDV-gestützte Auswertung eines Kreatinin-Anstiegs in Form eines Frühwarnsystems mit relativ einfachen Maßnahmen und der Unterstützung des Medizinischen Rechenzentrums bzw. IT umsetzbar. So erleichtert die Kennzeichnung von Patienten auf dem Laborbefund, z. B. mittels der Ausgabe „Akute Nierenschädigung“ (AKI-Alarm), die Frühdiagnose. Neue Marker wie Cystatin C haben Vorteile gegenüber der Kreatinin Bestimmung, sodass bei Verdacht des Vorliegens von Limitationen des Kreatinins bei einem bestimmten Patienten die zusätzliche Cystatin C Bestimmung zur bestmöglichen Abschätzung der glomerulären Filtrationsrate unter Nutzung aktueller Formeln erwogen werden kann.
