Zusammenfassung
Der Lichen sclerosus, der etwa 10-mal häufiger bei Mädchen als bei Jungen auftritt, ist vermutlich eine genetisch determinierte Autoimmundermatose. Extrakutane Manifestationen im Kindesalter sind eine Rarität. Die typische Manifestationsform ist die vernarbende Phimose bei Jungen und der anogenitale Lichen sclerosus bei Mädchen. Während bei Jungen die vollständige Zirkumzision mit einer Ausheilung der Erkrankung einhergeht, ist bei Mädchen die Prognose nicht so günstig. Neben einer kontinuierlichen Basispflege bedarf es einer antientzündlichen topischen Therapie mit Glukokortikoiden (Clobetasolpropionat bzw. Mometasonfuroat), evtl. in Kombination (off-label) mit Calcineurininhibitioren (Tacrolimus oder Pimecrolimus).
