Zusammenfassung

Ursache des fokalen Anfalls ist eine Funktionsstörung in einem umschriebenen Hirnbezirk. Diese relativ alte Definition des fokalen Anfalls ist auch heute im Grunde noch zutreffend, stellt aber eine Simplifikation dar, die der meistens komplexen Pathogenese und Pathophysiologie fokaler epileptischer Störungen nur unzureichend gerecht wird. Sie erweckt die Vorstellung sehr einfacher Zusammenhänge zwischen Ort der Anfallsentstehung, Anfallssymptomatik und EEG-Befund. Solche klaren Gegebenheiten bestehen z. B. bei fokalmotorischen Anfällen einer Hand mit einem lokalisationsentsprechenden kontralateralen Herdbefund im EEG und einer in dieser Region im MRT nachweisbaren Läsion. Anfallssymptomatik, EEG- und bildgebender Befund stimmen also überein. So einfach und übersichtlich stellen sich die Verhältnisse aber nur bei fokalen Anfällen dar, die direkt am primären motorischen, somatosensorischen oder sensorischen Kortex entstehen.
