Zusammenfassung
Die akute hämatogene Osteomyelitis (AHO) tritt typischerweise in den Metaphysen der langen Röhrenknochen auf. In zwei Drittel der Fälle sind Femur, Tibia oder Humerus betroffen. Zunächst kommt es zu einer hämatogenen Besiedelung des Knochenmarkraums und zur Etablierung eines lokalen Herdes. In über 80% der Fälle liegt eine Infektion mit S. aureus vor. Eine Ausnahme sind Kinder unter 2 Jahren, bei denen Streptokokken (Streptococcus pygenes, pneumoniae, Gruppe B) für über 60% der Fälle verantwortlich sind. Andere Ursachen einer AHO sind: koagulasenegative Staphylokokken (bei Fremdkörper assoziierten Infektionen), enterale gramnegative Keime (v. a. Salmonellen bei Sichelzellanämie), Anaerobier, Hämophilus influenca B (wobei die Infektionen seit Einführung der Regelimpfung stark rückläufig sind). Seltene Problemkeime wie Mycobacterium, Bartonella, Pilzinfektionen (Histoplasma, Cryptococcus, Candida) und Coxiella treten nur bei besonderen Risikofaktoren auf (Tab. 42.1).
