Rheumatisches Fieber und poststreptokokkenreaktive Arthritis

Zusammenfassung

Das rheumatische Fieber (RF) ist eine 2–4 Wochen nach einer Streptokokkeninfektion auftretende akute systemische entzündliche Erkrankung mit variablem klinischen Bild und Rezidivneigung. Es tritt bevorzugt jenseits des 4. Lebensjahres auf mit einem Häufigkeitsgipfel um das 10. Lebensjahr. Erkrankungen im jüngeren Kleinkindes- oder Erwachsenenalter sind Raritäten. Mit einer Inzidenz von ca. 1–5/100.000 ist die Erkrankung in industrialisierten Ländern selten, in Entwicklungsländern aber sehr viel häufiger. Bei Epidemien können bis zu 3% der Streptokokkeninfektionen von einem RF gefolgt sein. Seine Bedeutung hat das RF nicht nur wegen der akuten Erkrankung, sondern v. a. wegen der schwerwiegenden Folgeschäden an den Herzklappen, wobei diese Gefahr bei jüngeren Kindern am höchsten ist.
