Zusammenfassung
Die sekundär sklerosierende Cholangitis (SSC) ist eine cholestatische Gallenwegserkrankung, die gekennzeichnet ist durch Inflammation, obliterative Gallenwegsfibrose, biliäre Strikturen und schließlich biliäre Zirrhose. Im Gegensatz zur primär sklerosierenden Cholangitis geht der SSC fast immer ein identifizierbarer Auslöser voran. Ursachen einer SSC sind Ischämie der Gallenwege (durch Low-Output Syndrom, intraarterielle Chemotherapie oder Thrombose der A. hepatica nach Lebertransplantation), septischer Schock oder Inflammation i. R. von Polytrauma oder Verbrennung, anhaltende biliäre Obstruktion und Gallenwegstraumata i. R. von operativen Eingriffen (Engler et al. 2003; Ruemmele et al. 2009). Gerade die SSC nach Low-output-Syndrom oder septischem Schock (neuerdings zusammengefasst unter dem Begriff SC-CIP, „sclerosing cholangitis of the critically-ill patient“) stellt eine gravierende mittelfristige Komplikation der modernen Intensivtherapie dar. Ihre klinische Bedeutung ist erst in den letzten Jahren vollständig erkannt worden. Inwieweit häufig vorbestehende Lebererkrankungen wie die nichtalkoholische Steatoheatitis und eine begleitende Leberfibrose den Verlauf der SSC begünstigen, ist derzeit noch unklar.
