Zusammenfassung
Die Skaphoidfraktur ist der »Klassiker« unter den Handwurzelverletzungen. Wie bei vielen anderen Frakturen hat sich hier in den letzten Jahren ein deutlicher Trend von der konservativen hin zur operativen Behandlung vollzogen. Gründe dafür sind einerseits die Verfügbarkeit geeigneter Osteosyntheseimplantate sowie der Wunsch der Patienten nach schnellstmöglicher Rehabilitation mit kalkulierbaren Behandlungszeiten und -ergebnissen. Durch den heute weit verbreiteten Einsatz der Computer- und MR-Tomografie in der Diagnostik wurde die Beurteilbarkeit der Frakturmorphologie im Vergleich zu den Möglichkeiten der konventionellen Röntgentechniken auf ein neues Qualitätsniveau gehoben, welches die Voraussetzungen für eine differenzierte Therapie geschaffen hat. Trotzdem verbleibt eine nicht unerhebliche Anzahl von Patienten, die sich infolge nicht ausgeheilter Skaphoidfrakturen aufwendigen Pseudarthrosebehandlungen oder sog. Rettungsoperationen wegen irreversibler Sekundärschäden des Handgelenks unterziehen muss. Deshalb wäre eine Rücknahme der hohen und teilweise kostenintensiven Standards in der Diagnostik und Therapie ein Sparen an der falschen Stelle, denn bei Eintritt des »worst case« einer Pseudarthrose oder Handgelenkarthrose würde dies eine Verschiebung der Kosten mit hohem Multiplikator in die Zukunft bedeuten. Die konsequente Diagnostik und Behandlung der Skaphoidfraktur bewahren somit nicht nur den Patienten vor negativen körperlichen und sozialen Folgen bis hin zur Berufsunfähigkeit, sondern auch den Behandler vor späteren juristischen Anfechtungen.
