Zusammenfassung
Die Ursachen für das Ausbleiben einer gewünschten Schwangerschaft sind vielfältig und haben sich, wie das reproduktive Verhalten in den westlichen Gesellschaften, in der Vergangenheit erheblich verändert. Zur ersten Abklärung des unerfüllten Kinderwunsches gehört eine gynäkologische Anamnese mit detaillierter Regelanamnese, ein Basishormonstatus, ggf. die Abklärung eines Tubenfaktors und natürlich ein Spermiogramm. Bereits nach 6 erfolglosen Zyklen ist eine diagnostische Abklärung ratsam (▶ Kap. 1). Zu diesem Zeitpunkt beträgt der Anteil der idiopathischen, d. h. unerklärten Subfertilität eines Paares etwa 23 % (Snick et al. 1997; Abb. 23.1). Später, mit fortlaufender Behandlung eines unerfüllten Kinderwunsches und tieferer diagnostischer Erkenntnis, verändern sich die prozentualen Anteile der Diagnosen bei Subfertilität (Abb. 23.2).
