Zusammenfassung
Die Labordiagnostik dient der Erkennung einer klinisch relevanten Hormonaktivität (Funktionalität) von pankreatischen neuroendokrinen Tumoren (pNET). Gastrinome werden durch Messung des erhöhten Serumgastrins (falls erforderlich im Sekretintest) bei gleichzeitiger gastraler Hyperazidität diagnostiziert. Typisch für Insulinome sind symptomatische Hypoglykämien im Nüchternzustand (standardisierter Fastentest) mit einem erhöhten Insulin-Glucose-Quotienten im Plasma. Den wichtigsten Marker für die Verlaufsbeurteilung non-funktionaler und funktionaler pNET stellt das Chromogranin A im Serum dar. Die MEN1-Gendiagnostik ist bei Patienten mit pNET indiziert, wenn nach etablierten Kriterien der Verdacht auf ein MEN1-Syndrom besteht. Bei positivem MEN1-Mutationsnachweis wird eine genetische Familienberatung und Gendiagnostik empfohlen, außerdem ein regelmäßiges Nachsorgeprogramm, das obligate und fakultative Labordiagnostik beinhaltet.
