Pancreas divisum: Klinik, Diagnostik und interventionelle Behandlung

Zusammenfassung

Das Pancreas divisum kann in Einzelfällen zu behandlungsbedürftigen Erkrankungen und Symptomen wie einer rezidivierenden akuten Pankreatitis, einer chronischen Pankreatitis sowie abdominellen Schmerzen führen. Aktueller Goldstandard der Diagnostik ist die Sekretin-stimulierte MRCP. Die endoskopische Therapie mittels Minorpapillotomie, ggf. Gangdilatation und kurzfristiger Stenteinlage beruht auf dem Prinzip der intraduktalen Druckreduzierung und ist langfristig bei bis zu 70 % der behandelten Patienten klinisch erfolgreich. Die endoskopische Minorpapillotomie ist ein komplexes interventionelles Verfahren und sollte in Zentren mit einer hohen Expertise hierfür erfolgen. Wichtige Komplikationen sind Blutungen, Perforationen, Infektionen und die postinterventionelle Pankreatitis, deren Risiko durch eine vorübergehende Stentimplantation und die postinterventionelle Gabe von Indomethacin reduziert werden kann.
