Evidenz der operativen Therapie der chronischen Pankreatitis

Zusammenfassung

Die chronische Pankreatitis stellt eine interdisziplinäre Erkrankung dar, bei der unterschiedliche Therapiekonzepte miteinander konkurrieren. Zum einen interventionell-endoskopische und chirurgische Verfahren, zum anderen unterschiedliche chirurgische Verfahren untereinander. Dabei ist es in den letzten 20 Jahren gelungen, die Evidenzlage schrittweise zu verbessern. Bei dem Vergleich interventionell-endoskopischer und chirurgischer Verfahren konnte gezeigt werden, dass bei Patienten mit chronischer Pankreatitis und Gangokklusion im Pankreaskopf und Pankreaskorpus eine Drainage-Operation der interventionellen Therapie vorzuziehen ist. Ein weiteres häufiges klinisches Problem stellt der entzündliche Pankreaskopftumor dar. Hier konnte beim Vergleich unterschiedlicher chirurgischer Verfahren in mehreren Studien gezeigt werden, dass die duodenumerhaltende Pankreaskopfresektion und die Duodenohemipankreatektomie als mindestens gleichwertig anzusehen sind.
