Zusammenfassung
Ein erhöhter Alkoholkonsum ist für 70–85 % der Fälle verantwortlich und damit mit Abstand die häufigste Ursache der chronischen Pankreatitis (CP). An der idiopathischen CP (20–25 %) sind in annähernd der Hälfte aller Fälle prädisponierende Genmutationen ursächlich beteiligt. Alle anderen Ursachen der CP sind selten. Pathogenetisch ist die CP durch einen Umbau des Organs mit Verlust des funktionellen Parenchyms und Ersatz durch fibrotisches Bindegewebe charakterisiert. Wie es im Rahmen der verschiedenen Ätiologien von der initialen Schädigung zur chronischen Entzündung und zu den genannten Umbauvorgängen kommt, ist weitgehend unbekannt. Im Rahmen des Verlustes von Parenchym kommt es in Abhängigkeit der Erkrankungsdauer zu einer exokrinen und endokrinen Insuffizienz. Im Vordergrund steht für die Patienten allerdings meist eine ausgeprägte lokale Schmerzsymptomatik, die nicht nur durch die lokalen Komplikationen der CP, sondern auch durch eine pankreatische Neuropathie verursacht wird.
