Zusammenfassung
Die Therapie chronischer Schmerzen bei Suchtkranken ist kompliziert. Zu gleicher Zeit sind zwei unterschiedliche chronische Erkrankungen vorhanden. Begleitende psychische Erkrankungen sind in beiden Krankheitsgruppen häufiger als in der Allgemeinbevölkerung. Schmerzstärke und funktionelle Beeinträchtigungen sind erheblich, die Wirksamkeit starker Opioide ist vermindert. Typisch sind vielfältige psychische Auffälligkeiten und ein maladaptatives Coping mit passivvermeidender Grundhaltung. Die Compliance gegenüber multimodalen Therapieoptionen ist oft gering und mit Substanz- und Pharmakamissbrauch verbunden. Ein schmerztherapeutischer Erfolg ist bei nichttumorbedingten Schmerzen deutlich schwieriger erreichbar als bei tumorbedingter Ätiologie.
In früheren Jahren galt in der Suchttherapie das Paradigma der Abstinenz, ebenso war diese die Voraussetzung für eine Schmerztherapie. Das ist heute nicht mehr der Fall, es sei denn, die entgleiste Suchtkrankheit lässt einen sinnvollen Patienten- Arzt-Kontakt nicht zu. Durch die Zusammenarbeit eines Schmerzmediziners mit einem in Suchtfragen Erfahrenen (Psychiater, Psychosomatiker, Psychologe) können erschwerende Faktoren sowie ungünstige Nutzen-Risiko-Relationen verschiedener Therapieoptionen rechtzeitig erkannt werden.
Aufgrund des gleichzeitigen Zusammentreffens von Sucht und chronischem Schmerz müssen psychische Störungen intensiv in Diagnostik und Therapie eingebunden werden, was von den Betroffenen allerdings nur eingeschränkt akzeptiert wird. Die Therapie sollte sich darauf ausrichten, das Copingverhalten in eine aktive Richtung zu verschieben, die Compliance zu verbessern und die Akzeptanz gegenüber der Schmerzerkrankung zu erhöhen. Therapiekontrollen sind häufiger als sonst nötig, um im Falle fehlenden Erfolgs eine rechtzeitige interdisziplinäre Reevaluation durchzuführen und erweiterte Therapieoptionen zu nutzen.
