Zusammenfassung

Mit dem Begriff Neuromodulationsverfahren werden nichtdestruktive, reversible schmerztherapeutische Verfahren am peripheren oder zentralen Nervensystem bezeichnet. Diese beinhalten Stimulationsverfahren mittels extern applizierter oder implantierter Elektroden und die intrathekale Medikamentengabe. Diese Methoden haben heutzutage die destruktiven Verfahren weitgehend abgelöst. In den letzten Jahren wurde versucht, die Wertigkeit und Wirksamkeit dieser Verfahren durch randomisierte kontrollierte Studien – wenn auch ohne Verblindung – im Langzeiterfolg zu belegen. Grundsätzlich muss man bei der Indikationsstellung die Schmerzursache, Malignom-bedingte Schmerzen und Schmerzen nichtmaligner Ursache trennen. Gerade bei chronischen Schmerzen nichtmaligner Ursache müssen weniger invasive oder konservative Verfahren ausgeschöpft sein und die Patienten einer psychologisch-psychiatrischen Evaluation unterzogen werden, bevor invasive Verfahren zum Einsatz kommen.
