Zusammenfassung
Zahllose erblich bedingte Erkrankungen des Stoffwechsels induzieren diffuse zerebrale Funktionsstörungen. Als „inborn errors of metabolism“ führen die primären neurometaboli-schen Syndrome zu Funktions- und Strukturschäden im ZNS, die prototypisch als eine Enzephalopathie ohne fokale Betonung mit ausgedehnter Verteilung auftreten. Grob gegliedert in die hereditären und erworbenen metabolischen Erkrankungen werden die bei erwachsenen Patienten anzutreffenden angeborenen Stoffwechselstörungen und ihre Auslöser besprochen. Anzustreben ist, die umfangreichen Möglichkeiten der Labordiagnostik durch wesentliche Informationen aus Familienanamnese, bezüglich Organbefunden und körperlicher Stigmata einzugrenzen. Besondere Bedeutung kommt der Kenntnis spezieller Auslöser neurometabolischer Krisen (durch Fasten, Fieber, Nahrung, Trauma usw.) zu, auch und besonders in therapeutischer Hinsicht.
