New Trends in Shoulder Arthroplasty

Abstract

Total shoulder arthroplasty has become a routine procedure with reliable and reproducible results. Modular systems, which are based on increased knowledge of the anatomy of proximal humerus, allow the surgeon to re-create anatomy. The trend is to use more stemless humeral designs with metaphyseal fixation. On the glenoid side pegged glenoids are used more frequently and augmented glenoids have been introduced to manage posterior bone loss. One of the most important developments in the last decades has been the Grammont type reverse total shoulder arthroplasty. Despite specific fracture hemi-arthroplasty designs the outcome remains unpredictable and therefore reverse arthroplasty has been recommended in acute proximal humeral fractures in the elderly. Platform systems, which allows the surgeon to revise a failed anatomical arthroplasty to a reverse arthroplasty without stem removal, has been advocated to facilitate revision surgery.
