Temporo-Parietal Fascial Flaps to Improve Ear Reconstruction

Abstract

The use of fascial flaps is a good technical procedure for ear reconstruction. My first descriptions based on anatomical cadaver dissection supported reducing the number of surgical stages when performing ear reconstruction. I demonstrated that vascularization of the neighboring regions of the ear is supplied by the superficial temporal, posterior auricular, and occipital arteries. The information provided in my early articles elucidated the anatomy and provided useful information to perform reconstruction in a single operation, which was my goal since beginning to perform ear surgery. In fact, before my publications, several operatory stages were required to perform ear reconstruction. However, after performing ear reconstruction procedures, I concluded that it was possible to perform the operation in one or two surgical stages without employing fascial flaps. For that reason, since 1979 I have not used fascial flaps during primary ear reconstruction on congenital anomalies. I prefer to reserve them for three circumstances:

    Treatment of complications after ear reconstruction

  2. 2.

    Partial or total ear reconstruction after severe traumatic amputation

  3. 3.

    Secondary ear reconstruction when the local skin has been severely damaged
