Die ersten Klappenersatzoperationen erfolgten 1960 mit Hilfe von Kugelprothesen. Harken et al. (1960) führten den ersten Aortenklappenersatz, Starr u. Edwards (1961) den ersten Mitralklappenersatz durch. Der erste Homograft in Aortenposition wurde 1962 von Barrat-Boyes (1964), die erste gestentete Bioprothese 1965 durch Binet (Binet et al. 1965) implantiert. Ein Transkatheteraortenklappenersatz (TAVI) wurde erstmals durch Cribier 2002 erfolgreich durchgeführt (Cribier et al. 2002). In Deutschland werden in der Erwachsenenherzchirurgie zurzeit etwa 25000 Klappenoperationen durchgeführt (Aortenklappe = 11500, Mitralklappe = 5300, Trikuspidalklappe = 470; Aorten- und Mitralklappe = 1800; Klappe kombiniert mit aortokoronarem Bybass (ACB) = 11400, TAVI > 3500), die Tendenz ist steigend.
