Zusammenfassung

Bei der direkten Lymphographie wird nach Farbstoffmarkierung mit Patentblau ein Lymphgefäß am Fußrücken freipräpariert und punktiert, um das Lymphgefäßsystem nach Injektion eines öligen Kontrastmittels darstellen zu können. Man unterscheidet die lymphangiographische Phase (Lymphgefäßdarstellung während der Lymphographie oder unmittelbar danach) und die lymphadenographische Phase (Lymphknotendarstellung 24 h nach der Lymphographie).

Als diagnostische Maßnahme wurde die direkte Lymphographie früher zur Lymphknotendiagnostik bei Hodgkin- und Non-Hodgkin-Lymphomen sowie Lymphknotenmetastasen eingesetzt, dies ist heute obsolet. Als therapeutische Maßnahme kommt die Lymphographie zunehmend wieder zum Einsatz bei therapierefraktären Lymphfisteln inguinal, retroperitoneal und thorakal. Nach Lipiodolaustritt über die Fistel bildet sich Granulationsgewebe, was zum Fistelverschluss führen kann [1].
