Zusammenfassung
Muskelschmerz unterscheidet sich stark vom Hautschmerz. Bei Muskelschmerz treten keine Flexorreflexe auf, und die zentrale Verschaltung der Afferenzen von Muskelnozizeptoren ist anders. Muskelnozizeptoren werden durch Bradykinin und Prostaglandine sensibilisiert, die bei Muskelläsionen freigesetzt werden. Die Folge ist das Auftreten von Schmerzen bei leichten Reizen. Eine starke Aktivität von Muskelnozizeptoren führt zu einer Sensibilisierung von Hinterhornneuronen. Die Neurone reagieren dann verstärkt auf periphere Reize. Ein Merkmal von Muskelschmerzen ist die Übertragung der Schmerzen. Übertragung bedeutet, dass die Schmerzen entfernt von der Muskelläsion empfunden werden. Das Konzept, dass Muskelschmerz zu Spasmen im schmerzenden Muskel führt, ist überholt. Ein schmerzender Muskel wird zentralnervös abgeschaltet. Beim nichtspezifischen Rückenschmerz scheint die Fascia thoracolumbalis eine wichtige Rolle als Schmerzquelle zu spielen.
