Antikoagulation bei AV-Gefäßanschlüssen

Zusammenfassung

Folgendes Schema hat sich in unserer klinischen Praxis bewährt:

a. AV-Fisteln und AV-Shunts bedürfen bei physiologischen Kreislaufverhältnissen und physiologischen Gerinnungsverhältnissen keinerlei medikamentöser Beeinflussung der Gerinnung.

b. Eine primäre Antikoagulation mit einem INR-Zielwert um 3 propagieren wir ausschließlich bei Patienten mit:

- einer nachgewiesenen Hyperkoagulabilität,

- arterioarteriellen Interponaten und

- einer Hypotonie (RR < 80 mmHg systolisch) bei AV-Protheseninterponaten.

c. Das Vorgehen bei Thrombosen von AV-Gefäßanschlüssen richtet sich nach den intraoperativen Befunden bei der Thrombektomie.
