Zusammenfassung
Zur Fraktur des proximalen Oberschenkels kommt es am häufigsten durch Sturz auf die Hüfte, aber auch im Rahmen von Hochrasanztraumata, z. B. Verkehrsunfällen (»dashboard injury«), Polytrauma. Man unterscheidet per- bis subtrochantäre Brüche, Schenkelhals- und acetabulumnahe Brüche. Pertrochantäre Frakturen sind typischerweise mit höherem Alter und/oder Osteoporose assoziiert. Die Einteilung erfolgt durch die AO-Klassifikation und die Klassifikationen nach Garden, Pauwels, Pipkin und Evans. Diagnostisches sowie therapeutisches Vorgehen sind durch bildgebende Verfahren verdeutlicht. Die Frakturversorgung erfolgt fast immer operativ. Die insbesondere angewandten Osteosyntheseverfahren sind die kanülierte Zugschraubenosteosynthese, Prothesen- und Hemiprothesenversorgung, Marknagelosteosynthese und die dynamische Hüftschraube (DHS).
