Are circulating monocytes activated following blunt chest trauma?

Abstract

Despite therapeutic progress blunt chest trauma is still associated with a high morbidity and mortality. We were previously able to demonstrate the priming of alveolar macrophages following experimental blunt chest trauma. However it is unclear if an experimental lung contusion does induce additional priming of circulating monocytes. The aim of this study was therefore to elucidate the (I) function of circulating monocytes following experimental blunt chest trauma and to asses (II) posttraumatic changes in the concentration of mediators which act predominately on macrophages/monocytes in bronchoalveolar lavage (BAL) fluids and the systemic circulation.

Wistar rats were randomly allotted to 4 groups, 8 animals each. Three groups were subjected to a blast wave injury and sacrificed 10 min (TX-10m), 6 hours (TX-6h) and 24 hours (TX-24h) after the trauma, another group served as control. The circulating monocytes were isolated and cultured in the presence of LPS and the concentration of TNF-α, MIP-2 and IL-10 was determined in the supernatant. In addition the release of MCP-1, MIP-2, IL-10 und PGE into the bronchoalveolar compartment and into the systemic circulation was determined.

There was no difference in mediator release into the supernatant between cultured monocytes of trauma and control animals. As early as 10 minutes after a blunt chest trauma there was a 2 fold increase (p < 0,05) in the concentration of PGE2 and IL-10 in the BAL fluids. The concentration of MCP-1 and MIP-2 was significantly elevated in the BAL fluids of TX-6h and TX-24h animals. No difference between trauma animals and controls was evident in the blood plasma.

Taken together the results indicate that experimental chest injury (I) induces the priming of alveolar macrophages but not of peripheral circulating monocytes and (II) causes an early and considerable release of pro- and anti-inflammatory mediators into the bronchoalveolar (= local) compartment but not into the systemic circulation.
