Stellenwert von Regionalanästhesieverfahren

Zusammenfassung

Die perioperative Versorgung von Patienten in der Gefäßchirurgie stellt eine besondere Herausforderung dar. Meist handelt es sich um ältere Patienten mit einer generalisierten Arteriosklerose und zahlreichen Begleiterkrankungen. Hierzu addieren sich erhebliche operationsabhängige Kreislaufbelastungen und Volumenverschiebungen. Trotz Fortschritten in der modernen Anästhesie liegt die perioperative Letalität gefäßchirurgischer Patienten noch zwischen 1–4%. Wesentliche Strategien zur Senkung der perioperativen Morbidität und Letalität gefäßchirurgischer Risikopatienten bestehen in der sorgfältigen präoperativen Risikoeinschätzung und engmaschigen perioperativen Überwachung. Inwieweit sich durch den Einsatz von Regionalanästhesieverfahren eine Reduktion der Morbidität und Letalität gefäßchirurgischer Patienten erzielen lässt, wird aktuell kontrovers diskutiert. Zahlreiche Untersuchungen weisen auf positive perioperative Effekte insbesondere rückenmarknaher Regionalanästhesieverfahren hin.
