Zusammenfassung

Störungen der Erregungsbildung und Erregungsleitung spielen unabhängig von der Ätiologie der Grunderkrankung in der klinischen Kardiologie eine wichtige Rolle. Ca. 20–30% der Überweisungen in kardiologisch orientierte Praxen und kardiologische Krankenhausabteilungen erfolgt wegen „Herzrhythmusstörungen“. Berücksichtigt man nur „notfallmäßige“ Einweisungen bzw. Vorstellungen, so ist der Anteil noch höher. Die Symptome für den betroffenen Patienten reichen von Palpitationen über Schwindel, Synkope, Herzrasen bis hin zum Herz-Kreislauf-Stillstand. Die hieraus resultierende Funktionsstörung wird vorrangig durch die Herzrhythmusstörung selbst bestimmt. Die weitere Prognose hingegen ist in erster Linie von der zugrunde liegenden Grunderkrankung und ihrem Schweregrad abhängig.

Die ursächlichen Mechanismen der Arrhythmogenese sind ausführlich in Kap. 3 dargestellt, die Differenzialtherapie der verschiedenen Arrhythmien in den Kap. 40 und 51. Auf Querverbindungen insbesondere zu den Kap. 19 und 20 sei hingewiesen.
