Zusammenfassung

Der untere Blutglukosewert, der die Grenze zwischen Normo- und Hypoglykämie repräsentiert, ist nur ungefähr festgelegt. Für den Stoffwechselgesunden wird als Grenzwert nach zwölfstündi-gem Fasten morgens nüchtern 50 mg/dl für Männer und 40 mg/dl für Frauen angesehen (Althoff et al. 1984). Allerdings können Personen, die an niedrige Blutglukosewerte gewöhnt sind, z. B. Patienten mit Insulinom, aber auch etwa 30% der stoffwechselgesunden Personen diese Werte unterschreiten, ohne typische Hypoglykämie-symptome aufzuweisen. Zerebral vorgeschädigte Patienten oder Diabetiker mit chronischer hyperglykämischer Entgleisung zeigen dagegen entsprechende Symptome schon bei einer Blutglukose von etwa 70 mg/dl. Das Auftreten von Symptomen bei Hypoglykämie hängt somit nicht nur von der aktuellen Blutglukosekonzentration ab, sondern auch von der Höhe der vorausgegangenen Werte, von der Schnelligkeit des Glukoseabfalls und der Dauer der Hypoglykämie sowie der Effektivität gegenregulatorischer Mechanismen.
