Zusammenfassung

Veränderungen von Bewegungsabläufen sind bei Patienten mit affektiven Störungen ein aus der klinischen Praxis bekanntes Merkmal. Anders als das Erleben des Patienten sind motorische Phänomene der direkten Beobachtung zugänglich. Sie können objektiv erfasst und quantifiziert werden und sind relevant für differenzialdiagnostische und -therapeutische Entscheidungen. Es liegen vielfältige Hinweise für die prognostische und pathophysiologische Bedeutung dieser Krankheitszeichen vor. Jedes motorische Merkmal stellt die Endstrecke eines komplexen neurophysiologischen Prozesses dar, an dem eine Vielzahl zentraler und peripherer Funktionen und Strukturen beteiligt ist. Die Analyse motorischer Muster und deren Störungen lässt daher Rückschlüsse auf zentralnervöse Mechanismen und Funktionen zu und ergänzt Befunde zu psycho-biologischen Mechanismen affektiver Störungen (Aldenhoff 1997). Dieses Kapitel gibt daher einen Überblick über die Phänomenologie und Pathophysiologie motorischer Phänomene bei affektiven Störungen und deren Relevanz für Diagnostik und Therapie.
