Zusammenfassung
Die infektiöse Endokarditis betrifft am häufigsten die Herzklappen, seltener das murale Endokard, Septumdefekte oder AV-Shunts. Die Inzidenz beträgt ca. 2–7 auf 100.000 Einwohner [1]. Prädisponierende Faktoren sind angeborene oder erworbene Herzklappenfehler, kongenitale Vitien, Zustand nach Klappenoperation oder -ersatz, Mitralklappenprolaps mit Insuffizienz, Zustand nach abgelaufener Endokarditis, i.v.-Drogenabusus, intrakardiales Fremdmaterial sowie Grunderkrankungen mit Resistenzminderung. In den letzten Jahrzehnten haben besonders die Staphylokokkenendokarditiden, akut foudroyante Verlaufsformen, Kunstklappenendokarditiden sowie nosokomiale Infektionen stetig zugenommen [2, 16]. Ein besorgniserregender Trend ist ferner in der steigenden Zahl von Endokarditiden nach Body-Piercing zu beobachten [14]. Während früher die Mortalität unbehandelt 100% betrug, liegt sie heute immer noch bei etwa 20%, für die Prothesenendokarditis sogar bei 40%.
