Zusammenfassung

Die chirurgisch zu behandelnden Läsionen der Milz umfassen Verletzungen, hämatologische Erkrankungen und seltener fokale Veränderungen. Wenn früher bei einer traumatischen Milzruptur eine Splenektomie vorgenommen wurde, wird heute wegen der unerwünschten Folgen des Milzverlustes (»overwhelming postsplenectomy infection«) nach Möglichkeit die Organerhaltung angestrebt. Milzerhaltende Eingriffe stellen aber wegen ihrer hohen operativ-technischen Anforderungen eine echte Herausforderung für den Chirurgen dar. Die Indikationen zur therapeutischen Splenektomie sind bei Verdrängungssymptomatik durch sehr große Milzen und bei den Folgen des Hypersplenismus mit Anämie und/oder hämorrhagischer Diathese gegeben. Bei verschiedenen gutartigen und malignen hämatologischen Erkrankungen hat die diagnostische Splenektomie wegen raffinierterer diagnostischer Verfahren und hochwirksamer Chemotherapie an Bedeutung abgenommen.
