Zusamenfassung

Im Rahmen des Tumorstagings wird bei 6–10% der Patientinnen mit Mammakarzinom bereits eine Fernmetastasierung detektiert (Harris et al. 1993). Metastasierungsorte für das Mammakarzinom sind in 15–20% die Lunge und Pleura (Pataphan et al. 1988). Etwa 20% aller Tumorrezidive manifestieren sich an der ventralen Brustwand oder im Sternum (Crowe et al. 1991). Die Resektion von Lungen-, Brustwand- und Sternummetastasen sind heute ein standardisiertes Therapiekonzept an spezialisierten thoraxchirurgischen Zentren. In der Regel sind die Metastasen mit einer niedrigen Morbidität und Letalität technisch gut resezierbar. Allerdings sind lediglich 30% der Patienten mit einer Lungenmetastasierung für eine Resektionsbehandlung geeignet. Selektionskriterien für dieses Therapiekonzept lassen sich nur in einer engen interdisziplinären Zusammenarbeit zwischen Thoraxchirurgen, gynäkologischen Onkologen und Strahlentherapeuten entwickeln. Das Hauptproblem der Metastasenchirurgie besteht darin, dass mit lokalen chirurgischen Maßnahmen eine generalisierte Erkrankung therapiert werden soll.
