Thoracic Duct Ligation

Abstract

Chylothorax occurs as a result of (primarily iatrogenic) injury of the thoracic duct or as a result of increased capillary permeability (caused primarily by malignoma) in the pleura. Prompt surgical therapy avoids loss of protein, vitamins, electrolytes and lymphocytes; furthermore, with early intervention, pleural adhesions are still easily divided. The videothoracoscopic technique is proving itself in the closure of leaks or fistulas of the thoracic duct and for diffuse chylothorax in pleurodesis procedures.
