Ursächlich ist meist eine Aspirationspneumonie mit Lungenabszess. Die Alkoholanamnese ist in der Regel positiv, die Frage nach „Bewusstlosigkeiten“ wird auch oft positiv beantwortet, der Zahnstatus ist schlecht und die Zahntaschen sind infiziert. Entsprechend geht in der Regel ein Lungenabszess voraus (▶ Kap. 24 und 28). Ein Empyem bei AEP ist heute die Ausnahme.
