Epidemiologie und Versorgung von Gefäßpatienten in Deutschland

Zusammenfassung

Die Epidemiologie arterieller und venöser Gefäßerkrankungen wird erfasst durch die Anzahl erkrankter Patienten (Prävalenz) sowie die Rate neu diagnostizierter Gefäßerkrankungen (Inzidenz). Da Gefäßerkrankungen im Alter häufiger auftreten wird aufgrund des demographischen Wandels die Anzahl gefäßkranker Patienten weiter zunehmen. Hinzu kommen die steigende Prävalenz des Diabetes mellitus (in den USA bei 8%), die steigende Zahl von Dialysepatienten und die Folgen einer falschen Lebensführung oft erst nach vielen Jahren klinische Relevanz erlangen. Die Symptomatik arterieller und venöser Gefäßerkrankungen reicht von völliger Beschwerdefreiheit (z. B. asymptomatisches Aortenaneurysma) bis zur unmittelbar lebensbedrohlichen Komplikation (z. B. Aneurysmaruptur). ◘ Tab. 6.1 gibt einen Überblick über die Zahl gefäßchirurgisch relevanter vaskulärer Hauptdiagnosen in deutschen Krankenhäusern im Jahr 2009. Stationäre Patienten mit vaskulären Begleitdiagnosen oder Begleittherapien werden vom Statistischen Bundesamt nicht systematisch erfasst (z. B. Portanlagen bei Tumorpatienten, vaskuläre Komplikationen bei sonstigen Operationen/Interventionen, Patienten mit terminaler Niereninsuffizienz, Diabetes mellitus oder sonstigen Manifestationen der Arteriosklerose). Viele Gefäßpatienten werden mittlerweile ambulant behandelt. Hierzu gehört insbesondere die primäre Varikosis mit einer Gesamtzahl von ca. 350.000 stationären und ambulanten Operationen/Jahr in Deutschland. Während im Jahr 2000 noch 166.000 Varikosis-Operationen stationär erfolgten ist diese Zahl auf ca. 100.000 im Jahr 2009 gesunken. Weitere ambulante vaskuläre Diagnosen sind die Phlebothrombose und/oder Phlebitis, Dialysepatienten, das Ulcus cruris und die Claudicatio intermittens.
