Im Vordergrund der chirurgischen Aspekte bei hämatologischen Erkrankungen stehen Splenektomie und Cholezystektomie. Die Milz ist der primäre Ort des frühzeitigen Abbaus von entweder defekten Erythrozyten bei kongenitalen hämolytischen Anämien oder von Antikörper-beladenen Erythrozyten (bzw. Thrombozyten) bei Autoimmunerkrankungen. Sie kann dadurch kontinuierlich an Größe zunehmen und zu hämatologischen sowie mechanischen Beeinträchtigungen des Patienten führen. Durch vermehrten Abbau von Hämoglobin entstehen Gallensteine (Billirubinsteine), die bei Patienten mit hämolytischen Anämien oftmals schon ab dem Schulkindalter nachweisbar sind.
