Anorektale Fehlbildungen sind seit Aristoteles bekannt und wurden erstmals von Suranus und Paul von Aegina behandelt, wobei einfache Membranen über der Analöffnung gespalten wurden (Grosfeld 2006). Der Vorschlag, bei höheren Atresieformen eine Kolostomie durchzuführen, stammt aus dem Jahre 1710 von Littre. Entscheidende Fortschritte in der Behandlung des Krankheitsbildes erfolgten jedoch erst nach dem zweiten Weltkrieg, als Rhoads 1948 als erster den abdominoperinealen Durchzug zur Behandlung hoher Atresieformen und 1953 Douglas Stephens die sakroperineale Anorektoplastik empfahlen. Stephens war auch der erste, der die Bedeutung der Levatormuskulatur einschließlich der Puborektalisfasern erkannte und eine detaillierte Einteilung der verschiedenen Formen anorektaler Fehlbildungen vornahm, die nicht mehr nur der 1930 von Wangensteen und Rice mittels Invertogramm beschriebenen und von Ladd und Gross 1934 empfohlenen einfachen Einteilung in hohe und tiefe Formen entsprach (Stephens 1963).
