Lymphatische Neoplasien und ihre Manifestation im Knochenmark

Zusammenfassung

Vorläuferzell-Lymphome und Leukämien der B- und T-Zellen, die akuten B- und T-lymphoblastischen Leukämien, betreffen häufig das Knochenmark. Die weitere Differenzierung erfordert kombinierte phänotypische und molekulare Analysen, die nach aktuellem Kenntnisstand hier behandelt werden. Reifzellige Lymphome betreffen das Knochenmark zumeist sekundär. Ein kleinerer Teil der Lymphome nimmt von hier seinen Ausgang (z.B. lymphoplasmozytisches Lymphom), bildet umschriebene Tumoren aus (sog. primäre Knochenlymphome) oder ist auf Knochenmark und Milz beschränkt (Haarzellleukämie, splenisches Lymphom). Das Infiltrationsmuster m Knochenmark neben der immunhistochemischen Charakterisierung ist für die verschiedenen Lymphomtypen charakteristisch. In diesem Kapitel werden damit umfassende und aktuelle Kenntnisse und Strategien für die Diagnostik und Differentialdiagnostik lymphatischer Knochenmarksinfiltrate vermittelt, die an der Praxis orientiert sind und sowohl in der Primärdiagnose eines Lymphoms, als auch für Staging, Risikostratifikation und Therapie Bedeutung haben.
