Abstract
This chapter considers the laparoscopic Total Colectomy with Pelvic Pouch procedure. The chapter presents the major indications and contra-indications for the procedure as well as the pre-operative considerations. We present our approach to the operation, which consists of a laparoscopic subtotal colectomy, followed by colon extraction and ileal pouch creation through the right lower-quadrant port site. Pneumoperitoneum is then re-established and the proctectomy is completed laparoscopically and the pouch anal anastomosis performed under laparoscopic visualization. A loop ileostomy is sited at the right lower quadrant port site.
