Abstract

Optic nerve sheath meningiomas classically are seen in middle-aged or elderly females and present as slowly progressing axial proptosis and gradual loss of vision. In addition to the appropriate clinical presentation, the diagnosis of optic nerve sheath tumors falls heavily on the imaging findings, especially given the complexity of their location and the considerable morbidities that may be associated with biopsy. Cross-sectional imaging is an essential diagnostic tool and magnetic resonance (MR) imaging in particular, with the increasing clinical availability of 3-Tesla magnets, remains the modality of choice for the imaging diagnosis of optic nerve pathology. Computed tomography (CT), however, is not without its advantages and can often reveal characteristic findings to help make the diagnosis. Thus, knowledge of the MR and CT imaging characteristics of optic nerve sheath meningiomas, in conjunction with a suitable clinical context, is essential to accurate diagnosis and differentiation from other similar appearing lesions.
