Zusammenfassung

Grundsätzlich sind 2 Formen von Rhythmusstörungen behandlungsbedürftig. Nämlich solche, die zu subjektiven Symptomen führen, und Arrhythmien, die mit einer prognostischen Belastung des Patienten verbunden sind. Zunächst ist zu fragen, inwieweit das klinische Bild tatsächlich durch eine Rhythmusstörung bestimmt ist. Hier werden Arzt wie Patient häufig fehlgeleitet. Herzrasen, Palpitationen, Schwindel werden nicht selten fälschlich als arrhythmiebedingt gewertet. Untersuchungen mit Langzeit-EKGRegistrierung und Telemetrie haben gezeigt, dass Palpitationen in über 50% der Fälle nicht durch Arrhythmien bedingt sind. Umgekehrt verursachten tatsächlich vorhandene Herzrhythmusstörungen bei weniger als der Hälfte der Patienten eine klinische Symptomatik (Senges u. Czygan 1985). Zwischen den subjektiven Beschwerden und der tatsächlichen Präsenz von Herzrhythmusstörungen besteht daher nur eine unsichere Beziehung.
