Abstract

Osteoarthritis of the hip joint is a very common disorder among the elderly. In Germany, about 5% of the population above 60 years are suffering from symptomatic osteoarthritis of the hip. Typically, the disease is progressing slowly for years or even decades leading to a final state with severe pain and functional impairment of the affected joint. Consequently, the patients’ quality of life is markedly diminished. During the 1960s, the development of prosthetic joint replacement led to a pivotal improvement of prognosis for these patients. Currently there are about 100,000 total hip replacements (THRs) performed per year in Germany in patients with osteoarthritis of the hip. Although the procedure yields satisfactory long-term results in 80–90% of cases, there is still a considerable amount of variability. Results may be influenced by patient characteristics (e.g. age, co-morbidity or compliance) by characteristics of the prosthetic device (e.g. material, method of fixation) or by details of the procedure itself (e.g. antimicrobial prophylactics, duration of the procedure, postoperative rehabilitation measures). For example: with respect to different types of prostheses 5-year failure rates between 0% and 20% are reported [1].
