Früher war das Ziel bei der Frakturversorgung eine anatomisch perfekte, rigide Osteosynthese mit exakter Reposition aller Fragmente (Abb. 44.1). Eine primäre Frakturheilung wurde angestrebt. Zugschrauben kamen zum Einsatz, das Periost und Muskelgewebe musste zur exakten Reposition im Bereich der Fraktur entfernt werden. Die Folge war neben dem Ausbleiben der Kallusbildung vor allem die gestörte “Biologie„. Es kam zu einer verzögerten Heilung, auch Plattenbrüche waren nicht selten. Radiologisch war es schwierig, die fortschreitende Knochenheilung bildlich darzustellen.
