Familienstudien zeigen übereinstimmend, dass bei Angehörigen schizophrener bzw. psychotischer Patienten Sekundärfälle mit Schizophrenie gehäuft auftreten (1–16%); die Vergleichswerte für Angehörige Gesunder liegen deutlich niedriger (0–2%). Das Erkrankungsrisiko steigt dabei mit dem Grad der genetischen Verwandtschaft. In Familien nichtpsychotischer psychiatrischer Patienten (etwa bei unipolar Depressiven) findet sich eine solche Häufung von Schizophrenien aber nicht. Die Weitergabe der Erkrankung folgt keinem mendelschen Erbgang, was für genetisch komplexe Erkrankungen typisch ist.
