Abstract

The approach to treatment of rheumatoid arthritis is undergoing fundamental change, and the concept that early and aggressive suppression of inflammation and control of disease activity may slow the rate of disease progression is increasingly accepted [22]. Nonsteroidal anti-inflammatory and slow-acting antirheumatic drugs are indisputably the primary treatment modalities in rheumatoid arthritis (RA). However, persistent synovitis in one or a few joints despite general response to medical treatment is a frequent problem and requires specific attention. In rheumatoid synovitis, not all joints are equally affected or responsive to medical treatment, and the reasons for this are unclear. Factors such as anatomical complexity and weight bearing may explain why persistent synovitis is often present in the knee joint [53].
