Abstract
With the advancement of endovascular interventions, a large portion of aortoiliac occlusive disease is now amenable to balloon angioplasty and stenting. Nevertheless, standard surgical revascularization with aortobifemoral bypasses remains an important mainstay of the treatment of aortoiliac occlusive disease. Aortobifemoral bypass is a procedure that has stood the test of time and can be performed with low mortality and morbidity and excellent long-term patency rates. It is ideally suited for patients with severe diffuse bilateral iliac occlusive disease involving long segments (greater than 5 cm) of external or common iliac artery disease. It may also be the only option besides extra-anatomic reconstruction in patients with bilaterally occluded iliac arteries, aortic occlusion or unsuccessful attempts at endovascular revascularization.
